Is the issue that Wine simply doesn't implement the "uninstall this shortcut"
method in Windows uninstallers?  Or do Windows uninstallers rely on Windows
keeping track of what shortcuts it installed and doing it for them?

In the latter case, we should be keeping a cache somewhere of the shortcuts
Wine has created so we can uninstall applications cleanly.
