Did this just start happening with 1.3.21?  If so, could you please do a
regression test and confirm that it's my fault?

Is this without adding the hacks (stock 1.3.21)?  In my testing the background
music only works at the title menu, so if you've found a case where it works
in-game then I may be able to narrow down the source of the problem.
