implement WaitNamedPipe and DisconnectNamedPipe

This implementation of named pipes adds some support for WaitNamedPipe
and DisconnectNamedPipe. i've done some quick tests, but i haven't
tested WaitNamedPipe thoroughly. Errors and timeouts still aren't
handled properly yet.

ChangeLog:
* Implement WaitNamedPipe and DisconnectNamedPipe.
  Add a state for each pipe handle in the server.
  Create a socket on when the pipe is opened, not before.
