crtdll revisited

[Dimitrie O. Paun]

The msvcrt/ removal patch allows for a simpler fix to
the crtdll problem.

I don't include a ChangeLog because if you like this one,
you should commit it together with the msvcrt/ removal patch,
as it's part of the same logical unit of work.
