SHLWAPI thread functions

Hi,

More SHLWAPI updates.

This patch brings the shlwapi thread functions together, and
completes the implementation of the dll's semaphore functions. It
sneaks in the .spec updates for my patch "SHLWAPI_regstream" also.

It requires both the shlwapi.h patch sent previously, and Juergens
recent "patch shlwapi" update to be applied first. You will then need
to re-run configure after applying.

   Fix: SHGetThreadRef() calls AddRef(), @356 param count wrong
