more DirectSound COM fixes

Fixes more COM issues.

Added SecondaryBuffer and DirectSoundCaptureNotify
COM objects to get reference counting right.

Added more tests and restructured tests for easier debugging.

This still needs at least one more refactoring pass to get
all the interfaces right but it works well as is.
