dbghelp fixes

Rob Shearman sent me for review a couple of fixes to dbghelp. I added a 
few more fixes. Basically, this patch should allow to:
- fix several issues related to module handling ELF and PE (Rob)
- finish to manage properly debug info for global variables and exported 
symbols (from PE or ELF shared objects)

The net result is the ability to see (in backtrace for example) some 
addresses in PE or ELF modules (even if no debug information is present).
