Toolbar: Redraw Fixes

Hi,

This patches fixes a number of redraw issues with toolbars. I can see no
reason for differentiating between buttons that have text and those that
don't. As far as I can tell, they should all have their background
repainted to render correctly. Additionally, whenever a message forces a
recalculation of the toolbar it should also cause the background to be
repainted.
