implementing C++ name undecoration in msvcrt

this is more or less a complete rewrite of what's in winedump. However, 
winedump's implementation is rather buggy and was lacking lots of 
elements (template support...)
This should fix it to some (large) extent.
If someone's willing to back-port the code to winedump, feel free to do it.
Note: we need the next patch to msvcrt type info management to get wine 
tests to succeed again
