USER: On creating 16bit windows convert null instances to current process (Oops resend, FIXED)

Just before I sent the last version I renamed a variable but didn't 
correct all the places where it was used, but it still compiled because 
there was a variable with the old name earlier in the function. If you 
apply the last version of the patch it will crash. :P

Changelog
  Fixed instance = 0 not converted to the current process when making a
window. Broke menus in some apps. (Thanks to Mike)
