[OLE #39] Change Stub Manager / Apartment Locking Rules

Hi,

A reference to the enclosing apartment MUST be held whilst a reference 
to the stub manager is held, otherwise the stub manager could reference 
freed memory from the apt member. It also fixes up the same apartment 
unmarshal test so the tests should actually start testing the code. If 
they had been working, the past weeks worth of patches would have fixed 
todos in the tests.
