[OLE #53] Cleanups

Changelog:
- Use InterlockedIncrement for the ipid counter instead of a critical 
section (suggested by Mike Hearn).
- Remove a line added by a bad merge.
- Implement RemUnkStub_Disconnect.
- Remove all of the RPC disconnect code.
