winmm: another round for unicode migration

this patch moves (almost) the rest of winmm to handle correctly the MM drivers 
as unicode drivers (for the 32 bit part)
mainly, the interface to 32 bit MCI driver is now unicode. The patch rewrites 
the winmm internals to cope with that, as well as all our builtin MCI drivers
the patch incidentally fixes a couple of corner cases (32 bit driver interface - 
like opening - is now unicode ; and it removes a couple of wrong A -> W calls).

(I bzipped the patch as it's quite big > 300K, partly because all the MCI 
command tables had to be rewritten too)
