[wined3d] move gl surface distription onto sub-structure.

Hi,
  This patch moves parameters relating to opengl on
the surface structure on to a glDesciption member.

Level and Target have been removed from LoadTexture,
and the dependency on surface->device has been
reduced.

A couple of compiler warnings in d3d9 have also been
fixed.

This patch doesn't get anything new working, but it
does help with encapsulation.
