Do not always fallback to oss if an alternate sound system is specified

I'm trying to test Alsa, and Wine falls back to OSS if the
Alsa init fails, even if I've specifically excluded oss.
This seems wrong to me, so:

   Do not fallback to defaults if a driver, mapper, or midi
   is specified in the registry; consolidate MMDRV_Init()
   into a single function.
