WGL: unify pixelformat code

Hi,

This patch unifies the WGL/GLX pixelformat conversion code and makes it easy to add additional (compatible) pixelformats.

A list of GLX/WGL pixelformats is created when it is needed and accessed by WGL functions using ConvertPixelFormatWGLtoGLX and ConvertPixelFormatGLXtoWGL.

My next pbuffer/multisampling patches will depend on this patch.
