[AppDB] (resend7) Allow maintainer requests on version submission

Add a checkbox so the user can send a maintainer request along with a new 
version.  The request is automatically processed and hidden from the 
maintainer queue, just as with super maintainer requests on app submission.

When submitting a new application, the form displays a selection of radio 
boxes, allowing the user to choose between no maintainer, maintainer and 
super maintainer.

Resend: Some licence-related code sneaked in the last time
