WineD3D: p8 improvements 2/2

Hi,

Use a fragment shader for P8 palette conversion. This is the final step to make StarCraft very smooth in case of DirectDrawRenderer=opengl combined with RenderTargetLockMode=readtex. Again the shader is only used for P8 render targets.
