[1/2] shlwapi/tests: Fix the todo blocks in the SHCreateStreamOnFile tests. [try 3]

This patch addresses all of the todo blocks in the shlwapi/istream.c
tests. It represents the minimal set of changes needed to make them
pass.

ChangeLog
1 - the original version, split out over 3 patches
2 - consolidated the 3 patches into a single patch
3 - use CREATE_NEW when STGM_FAILIFTHERE is used with bCreate==TRUE.
NOTE: I have kept the S_OK return for both Read and Write when they
are passed a null pointer, as this is what the tests are implying is
returned in that case.
