quartz: Obtain the duration using the index in avisplitter

I'm still working on seeking but that seems like a bit more work.

It parses the index to obtain the duration. Strictly speaking this is
not needed for legacy indexes, but I'm doing it anyway to verify I'm
doing it correctly. Seeking will use the same math so it needs to work
right. :-)
