[PATCH 00/21] WinHelp enhancements / try #2

The following serie implements the second and last round of code consolidation for WinHelp before the move to Richedit for rendering.
In details:
- better code for window management
- history and back navigation has been enhanced
- several bug fixes in UI (caption, popup color, link, raising windows),i
  in .hlp file parsing (multi-column) and in macro handling (JumpId and JumpKeyword)
- infrastructure for handling links within a page (and not only at the top)

Version #2: including fix for Alexandre's and Juan's comments
