msxml3: Implement ownerDocument (try 2)

Hi,
    This patch changes all the create_xxx functions to include an 
IXMLDOMDocument pointer, which is then stored as part of the node . 
Since ownerDocument returns a "read-only" pointer, AddRef/Release is not 
used on this pointer.
