wininet: Fix incrementing lpdwIndex in HTTP_HttpQueryInfoW

HttpQueryInfo should only advance lpdwIndex when the
HTTP_QUERY_FLAG_REQUEST_HEADERS flag is set.
Also added conformance tests.
