[PATCH 1/6] hhctrl.ocx: Fix HTML Help Index tab with alternative 'keyword' style.

    Some HTML Help "Index" documents use a different formatting for
each index entry.  In this alternative formatting scheme the 'name'
parameter appears twice instead of having a 'keyword' parameter and a
'name' parameter.  The attached patch fixes this problem (Bug #21603).
