mmdevapi: Add support for openal disconnected extension

If pulseaudio dies, the device will become disconnected. Detect this 
case, and try to reconnect to pulseaudio whenever it is possible. :)

Also add device validation where needed, to prevent spewing openal 
errors when disconnected.
