wininet(1/4): Rename a couple members to reflect their usage, and remove redundant comments

The comments seem to indicate the meaning of dwUnknown4 and
dwUnknown5, and are confirmed by publicly available information on the
format of index.dat.  More on publicly availabe information in a
minute, so as not to confuse the issue with this patch.  Since the
member names changed aren't used yet, renaming them should have no
more impact than a comment.
