wininet(1/10): Add more tests for CommitUrlCacheEntry

This patch series tidies up URL cache entry management significantly,
resulting in fewer bogus cache entries and fewer duplicate files lying
around.  Unfortunately the existing cache directory is pretty much
hosed:  use counts are invalid and untracked files remain on disk, so
there's no way to recover state cleanly.  I'd recommend users purge
the contents of their cache directories, either manually or using the
control panel once Alexander Scott-Johns's patches get in.  There
isn't any harm in doing so, because nothing actually uses the cache
anyway.
