[3/3] msxml3: Allow to create dummy node map for <?xml ?>

Office 2007 depends on that apparently. To support this properly we need 
a customizable named node map implementation that allows some tricks for 
attributes enumeration (processing instruction can't have attributes, 
but <?xml can in msxml).
