[1/3] d3d8: Remove the palette implementation methods (try 2)

Changes: Dropped the patches that prevented P8 surfaces/textures from being
created. Wined3d will reject creation of P8 textures. P8 surfaces can be
created, like on Windows, and they're just as useless as on Windows.
I'm still removing the d3d8/9 style palette API because this reduces the
complexity of palette handling in wined3d a lot. If there is a way to use P8
surfaces on Windows that I haven't found we can implement it with the ddraw-
style palette handling in wined3d.
