po: Use '+' when specifying a modifier + accelerator key combination.

This restores the translations that were fuzzied by the standardisation on using '+'. This also adds new translations for the 'Close\tCtrl+F4' variant.

In most places we had translations for 'Close\tAlt+F4', so the 
translations for 'Close\tCtrl+F4' are pretty easy to guess (given the   
other translations for 'Ctrl').
