Do you have PulseAudio installed? Wine sometimes conflicts with it, so try removing it if you have it installed.

You could also mess around with the audio settings in winecfg, for example, try using a different sound driver here (but only one at a time!).

Finally, what kind of sound card are you using? An USB one? Are you sure ALSA properly detects it in the first place?
