slightly cleaned up

slightly more cleaned up version (GetTextExtentPointI -> GetTextExtentExPointI,
removed WineEngGetTextExtentPointI in favour of WineEngGetTextExtentExPointI).

it is, of course, still crappy (I am still duplicating most of
GetTextExtentExPointW).  And I still have no idea what it's actually doing :) 
(for example, does the tmBreakChar even have a glyph index?)
