Specifically the DLL allows itself to be unloaded whilst the Internet
Explorer_Hidden window is still running.

General question - why do we even bother refcounting DLLs in Wine? The resource
savings are tiny in comparison to the human cost of debugging mistakes like
this.  DllCanUnloadNow might have made sense in Windows 95 but I doubt it's
worth the hassle these days
