Crash on exit trace

The crash on exit bug is still in 1.23.  Attached is the tail end (after all
the D3D surface fixmes) of the console output from a recent session of mine
that didn't quit properly.  Wine brought up a window complaining that the
program had an error, but I was able to close the window, and wine quit
correctly after that.
