Adds missing VGA memory models (adds all 256 color mode support)

Adding missing memory models which extends previous VGA VESA text patch to all
256 color modes. Will shrink mode 19 rendering to the correct width/height
(instead of double size) and thereby fixes bugs in drawing all other 256 color
modes. This was also submitted to the repository and has been waiting
acceptance, but is posted here as well for convenience.
