Patch: Use kernel support for thread affinity when available

It turns out I'd already written a patch for thread affinity, but it was
rejected along with the process one, probably because the process one needed
work.  It also doesn't compile on MacOS, but as far as I know there isn't a
similar system call there anyway.  Does it address the issue for you?
