Hi, Hans.

Regarding comment 6 it's not clear where second declaration comes from. Was
another one added as PI node? libxml2 has a switch for saving context to skip
declaration on file start. I agree that it's better to rewrite saving
functionality, but at this point I need to know where another '<?xml..' comes
from...
