For me, this can happen every time I play 'offline' within cp_dustbowl, default
settings (and by default I mean, I keep the 15 bots, always let the computer
choose my character, etc).  The first two stages are fine, and I can complete
the third stage without getting the error.  But, if I remain in the third stage
for any amount of time (gain the first control point and then hang out), I'm
bound to lose fps and eventually time out.  When I time out, I return to the
TF2 menu and from this point, I can relaunch and restart an offline session. 
However, I am more prone to see the behavior earlier on (like in the first
stage as I connect).

Originally, I thought the five minute warning was triggering the behavior, but
now I have had the drop in fps well before the warning and also after the
warning. 

The behavior seems to occur within 20-30 minutes of play.
