My icons patch has been accepted and I've written and submitted a patch to
deduplicate associations in some cases
(http://source.winehq.org/patches/data/68621). To try the latter you'll have to
delete Wine's cached associations by running:

wine reg delete HKCU\\Software\\Wine\\FileOpenAssociations
