[Bug 23622] Severe bugs make displaying .chm files a disaster

I finally was able to download the .cab files, applied the patch to the current
git and am happy to report that our help files are displayed correctly and
internal/external links appear to work correctly.  Is there anything else that
needs to be tested?

Sad to say that icons are not displayed in the left side contents view.  What's
the deal with that?  I assume this has nothing to do with the latest gecko and
associated changes.
