winmm: Chain two ACM conversion streams together to support more formats

Here's a patch which implements the ACM chain I mentioned above, although your
problem might already have been solved by Henri's plughw patch.

Susan, can you check if Henri's patch 232893e0832bf5082285995303c482d50570e5ff
solved the issue?  If not, can you apply this patch and try again?
