New database dialog

The following dialog windows:
- create database
- delete database
- backup
- restore
are displayed incorrectly - the panel that should display dialog controls is
empty

To reproduce:
- start application
- connect to server
- navigate to Databases
- in context (right-click) menu select New database

Prerequisites:
- winetricks dotnet20
- recent version of windows in winecfg
- running instance of sql server (sql server 2000 is runnable in wine, see
appdb for details)
