It looks like you have vertex and pixel shaders disabled, perhaps through an
application specific registry key. For some reason disabling those makes it
much easier to reproduce this bug, although I did manage to make it crash
eventually without those settings just by playing through the game for a good
while.

Does the attached patch make it any better?
