Patch to avoid thrashing the pixel format when the game didn't set one

This patch may help with the pixel format thrashing issue that affects PoE and
Anno 1602.  I don't expect it to help with CS:GO.  I'm not sure about the
others since I don't know exactly what's going on with them.
