It is a duplicate of bug 38982 that is a duplicate of Ubuntu BUG
https://bugs.launchpad.net/ubuntu/+source/ubuntu-release-upgrader/+bug/1478844
Problem with the Linux 3.13.0-59-generic x86_64 kernel.
