At least on NVIDIA hardware you can downscale from a larger resolution to a
smaller one, but it doesn't always look great and it might be confusing for
users.  I'd recommend making it an option that's disabled by default.

For the refresh rate, just reporting the monitor's native refresh rate for all
available modes would be a good first start.  If you like, you can try to do
something fancier like choosing the largest mode that matches the target
refresh rate if multiple are available, and using the availability of
particular refresh rates to choose which refresh rates to report to the Windows
side.
