Did you mean HKCR instead of HKCU?

That is expected. The key ".pdf" does already exist. You can see it in the long
list of keys present in HKEY_CLASSES_ROOT.

I also tried adding a new key (".doc") to HKCR and closed regedit. I then
waited a few seconds and reopened regedit. The new key was also present and
visible.
