blackscreen workaround for staging-2.21

This patch works around the blackscreen problem and makes GTA5 start up every
time. It is probably not the right way to do it (really the bind flags should
come from DXGI_SWAP_CHAIN_DESC.BufferUsage) but it lets you at least play GTA:
5 without having to Alt+Enter a whole bunch of times.
