It's not shipped with .NET Framework 4.0 SDK, only in .NET Framework 2.0 SDK.

'Mdbg' from .NET Framework 4.0 SDK and your separate example download worked
even with very old Wine versions. I've tested with Wine 1.4 and Wine 1.6.2:

Be aware when using Wine built with (LLVM) MinGW cross-toolchain you might run
into bug 48417 ("Wine 32-bit builtins in PE format occupy low address space
range, preventing non-relocatable native executables from being loaded") when
trying to install .NET Framework 2.0 SDK ('winetricks -q dotnet20sdk' fails).
