Unfortunately we get different failures on different hardware, and it's
difficult to figure out what's happening without the exact same hardware. There
could be Mesa problems too.

As for the fixmes in blend_attachment_from_d3d12 and
shader_sm4_read_instruction_modifier, there were probably some tests back at
release 1.2 which used these, with tests marked todo. I thought tests with
LogicOpEnable were added only recently, but there are some much older ones.
